RN/LVN/PTA/COTA – CLINICAL CARE LIASION
JOB GOAL: The Clinical Care Liaison is responsible for the clinical assessment of referred patients for home care services. This assessment is meant to determine clinical appropriateness for home care, to ensure the ability to meet the needs of the patient and to prepare the team for the admission. The Clinical Care Liaison is also responsible for developing business partnerships generating home care referral growth by making effective sales contacts, calls, presentations and participating in Multidisciplinary Team meetings. Serves as the agency staff representative with physicians, facilities, discharge planners, and community agencies. Responsible for the overall development and referral growth of assigned territory
Essential Functions:

Develops, implements, and evaluates quarterly and annual territory plans to achieve growth goals and to implement agreed upon strategies and actions.
· Develops business relationships with providers by making effective sales calls and presentations to targeted referral sources.· Maintains professional and clinical knowledge of provider services. Acts as liaison between referral partners and agency to communicate information about new products, programs, and service delivery.
Meets with patients and families to discuss home care services, individual needs/concerns, and manage expectations, as needed.
Takes the initiative in building relationships within the Intake department and with the appropriate patient care resources staff to assure the establishment of effective communication with referral sources and internal stakeholders.
Updates and maintains customer and competitor profiles, logs daily call activity, and documents plans for follow up activities with referral sources.
Serves as agency staff liaison with physicians, facility nurses, social workers, discharge planners and community agencies; works with various community agencies to facilitate the delivery of appropriate services for potential patients.
· As requested by the Director of Business Development, participates in marketing efforts for the purpose of educating the healthcare community about home health programs, including physician calls, lunch and learns, health fairs, speaker’s bureau, and CEU programs for healthcare providers.· Prepares monthly tracking reports on referral sources and keeps agency informed of key marketplace changes relating to providers and competitors.

· Utilizes current Agency and/or department specific software to complete assignments.· Spends 90% of time in referral settings conducting sales calls, client’s supervision, coordinating the transition of care, Home Health Aide Supervision or completing clinical assessments.
Able to have clinical knowledge of competencies and capabilities of the Agency team’s clinical and rehabilitation product to accurately assess a patient and determine appropriateness for home care.
· Communicates with the Agency clinical and/or operations teams regarding admission preparedness and/or concerns pertinent to the delivery of care to the patient.· Obtains accurate information from hospital or referral source case managers, physicians, discharge planning staff, family/patient/caregiver as needed to prepare the Agency team for the admission.
Supervises Home Health Aide staff; Documents supervisory visits and all communication regarding patient care needs and pertinent care delivered in the company EMR.
Attends Agency, Company and/or Referral Source administrative meetings as needed and/or indicated.
Reports and investigates complaints or incidents per policy and documents appropriately in the EMR.

Requirements:
Education:
Bachelors Degree Preferred
Required: LPN/RN/COTA/PTA
Qualifications:
Strong oral and written communication skills, Excel, Word, and CRM management. Excellent presentation skills.
Experience:
3-5 years in patient care, sales, or related field; demonstrated proficiency in-home care marketing/sales preferred.
Physical Demands:
Regular local travel (weekly) in all reasonable weather conditions. Primarily medium in nature with exerting up to 35 pounds of force to lift, carry, pull, or otherwise move objects; involves sitting, walking, standing, driving, talking, hearing, and reaching.
Working Conditions:
In and out of offices, referral sources, and care throughout the workday. Exposure to disagreeable conditions, including odors, infection, illness, and physical contact with disruptive patients.
Work Schedule:
Must be flexible to accommodate the completion of tasks, operational needs, and referrals.
